Application Form For Islamic Divorce Certificate
Please complete the application on both sides of the paper and call the Masjid to make an appointment to meet the Imam.
Bring the completed application with you to the Masjid to have it reviewed.
If everything is in order, the Islamic Divorce Certificate may be issued immediately at the first appointment.
If there is information pending, then the Certificate will be issued upon the completion of all required information.
The cost for this service is $200

A Woman Applying To Terminate The Marriage
If a woman is applying to religiously terminate her marriage, then she must also submit proof of the legal termination papers if applicable. 
She must also provide the Masjid Office with a valid phone number or email of the husband so that he may be contacted and made aware of and included in the process.
